13-year-old girl with previous abdominal surgeries for a liver tumor 6 years ago, she develops portal cavernomatosis, portal hypertension with repeated digestive bleeding refractory to medical and endoscopic treatment, multiple hospital admissions. Abdominal tomography with several vascular anomalies and newly formed vessels. After the multidisciplinary assessment, we decided to face the challenge of attempting a portosystemic bypass. Surgery of many hours!!! We managed to perform a spleno-cavus shunt using a goretex prosthesis. Postoperative period without incident and discharge on the 6th day.
